Skilled nursing

Home health nurse responsibilities include assessing patient’s condition during every visit and chart the observations, perform evaluation tasks, including vital signs and medications review, maintaining patient medical histories, observing reactions or side effects. Finally they advise patients and their families on various health conditions.

Home health aide

A home health aide’s responsibility is to help the patient with personal care including baths, oral hygiene, shampoos, changing bed linen, dressing, toileting activities. Home health aide can also assist in feeding the patient. At the same time home health aide is trained to read vital signs, including pulse, temperature, respiration and blood pressure.

Medical social worker

Medical social worker are professionals who assess the psychosocial status of patients related to the patient’s illness and environment, carry out social evaluations and communicate the findings to the registered nurse. Medical social workers also serve a liaison between patients or families/caregivers and community agencies.

Speech therapy

Speech therapists are an essential part of home health care model who are responsible for assessing, preventing, diagnosing and treating speech disorders – including language, communication, cognitive and swallowing disorders.

Occupational therapy

Our occupational therapists help the patient to make small adjustments to improve, regain and develop the ability to complete activities the patient does on daily basis and improve the quality of life.

Physical therapy

Physical therapists help patients regain as much motor function as possible after injury or surgery. Physical therapists instruct patients and families/caregivers in the use and care of therapeutic appliances.
